The grant will continue UA researchers’ investigation of the benefits and harms of selenium supplementation, which could clarify conflicting information about colorectal cancer and diabetes risk in certain populations.

TUCSON, Ariz. – Personalized medicine — health care tailored to the individual patient — not only involves the use of prescription medications but vitamins and supplements as well. Determining who could benefit from selenium supplementation and who may be at risk could help guide health-care providers in making recommendations to their patients, particularly given that as many as half of American adults take a dietary supplement of some kind.
In response, researchers at the University of Arizona Cancer Center are delving deeper into clinical trial data on the benefits and harms of selenium supplementation. The team seeks to unravel the mystery of why some clinical trial participants experienced a reduction in recurrence of colon polyps that could lead to colorectal cancer, while some older participants had a slight increase in diabetes risk.
UA Cancer Center member Elizabeth T. Jacobs, PhD, professor and director of the epidemiology program at the UA Mel and Enid Zuckerman College of Public Health, is principal investigator for the project, which is funded by a $2 million grant from the National Cancer Institute through June 2022. Dr. Jacobs’ team will examine how selenium interacts with other components of a person’s diet to affect colorectal cancer risk and how genes might mediate the link between selenium and colorectal cancer risk.
“We’re not sure which genes affect response to selenium,” Dr. Jacobs said. “We’re in a unique position to be able to study this.”
“The research funded by this grant has the potential to benefit patients worldwide,” said UA President Robert C. Robbins. “Since much of the foundational investigation of the links between selenium and cancer has taken place at the UA Cancer Center, it is appropriate that our researchers continue pursuing this work. Three past major clinical trials by UA Cancer Center researchers have investigated the link between selenium and cancer, with mixed results. The Nutritional Prevention of Cancer Trial, a study of selenium supplementation for skin cancer prevention published in the Journal of the American Medical Association (JAMA) (December 1996), found that selenium supplements did not significantly reduce skin cancer incidence but did reduce colorectal cancer risk in participants with selenium deficiencies. The study was led by the late Larry C. Clark, PhD, MPH, who was a UA Cancer Center member and associate professor of epidemiology with the UA Zuckerman College of Public Health at the time of his death in March 2000. He also was an adjunct associate professor of nutritional sciences in the UA College of Agriculture and Life Sciences.
The Selenium and Vitamin E Cancer Prevention Trial (SELECT), a National Cancer Institute study that looked at prostate and other cancers, found that selenium supplements had no effect on colorectal cancer risk. Principal investigator for the UA Cancer Center site, one of more than 400 sites participating in the study, was Frederick R. Ahmann, MD, UA professor of medicine and surgery. Results were published in JAMA (January 2009).
The results of the 12-year Selenium and Celecoxib Trial, led by UA Cancer Center Deputy Director Peter Lance, MD, were published in 2016 in the Journal of the National Cancer Institute. Selenium supplementation was not associated with an overall reduction in colorectal polyp risk, and in men seemed to raise the risk for developing multiple polyps. However, a closer look at the data revealed the “heterogeneity of treatment effect,” in which different types of people responded differently to the treatment depending on factors like age and medical history. Among trial participants who took selenium supplements, those who had advanced colorectal polyps at the beginning of the study were 18 percent less likely to see a recurrence. This result raises the possibility that supplementation could be modestly beneficial in people whose history of advanced colorectal polyps puts them at increased risk for developing colorectal cancer.
The UA Cancer Center team also found that trial participants who were 63 years of age or older saw a slight increase in Type 2 diabetes. Therefore, while selenium might benefit some people, its effect might be neutral in other people and could be harmful in an older population.
“Given the popularity of dietary supplements, it is important that the public have reliable information about their safety to make the best decisions for their health,” said UA Cancer Center Director Andrew Kraft, MD. “Dr. Jacobs’ new study will go a long way toward clarifying questions surrounding the benefits and drawbacks of selenium supplementation.”
“This study is important and necessary, as it may help explain the conflicting results about the benefits and potential harm of selenium in previous studies,” said Iman Hakim, MD, PhD, MPH, dean of the UA Zuckerman College of Public Health.
The UA Cancer Center has a long history of investigating the effects of selenium on cancer risk. In addition to the previously mentioned large clinical trials, smaller clinical trials have explored the association between selenium supplementation and prostate cancer, beginning under the direction of Dr. Clark and completed by a UA Cancer Center team, including Dr. Ahmann, Amit M. Algotar, MD, PhD, and Steve Stratton, PhD. Selenium was found to have no effect on prostate cancer risk.
When the first large clinical trial investigating the association between selenium supplementation and cancer risk was being carried out by Dr. Clark at the UA Zuckerman College of Public Health, Dr. Jacobs was working on the study as a first-year graduate student. The results, showing that selenium supplements appeared to reduce risk for colorectal, prostate and lung cancers, attracted a flurry of media attention.
“After Larry Clark’s study came out, he went into Costco and all the selenium was off the shelves. People had bought it up,” Dr. Jacobs recalled. “Now, so many people are taking selenium. It is really important to find out whether taking these supplements is a good idea or not.”
Speaking to the idea that supplements don’t carry risks, Dr. Jacobs said, “We have a definite belief that dietary supplements are safe no matter how much of them we take. But we’ve shown repeatedly that that’s not the case. When it’s in food, it is generally safe. The problem is when we start taking really high-dose selenium and other nutrients.”
Until more studies are completed, Dr. Jacobs advises consumers to be cautious. “It looks like selenium is unable to prevent cancers overall,” said Dr. Jacobs. “We are not certain about the potential for harm, and that is what we are trying to find out.”
Reflecting on her involvement with the first large selenium trial in the 1990s and her subsequent career as a researcher, Dr. Jacobs said, “I’ve basically gone full circle back to what I started out in when I came here. I did a lot of different types of research and now we’re back to selenium. It’s 20 years later, and we still have these questions. That’s science — we’re still trying to dig through this.”
